Under 200 sq ft it is a site plan showing impervious area; over 200 sq ft it adds a grading plan and a drainage study by a Kansas licensed engineer. It is applied for apart from the building permit and has to be in hand first, so it goes in the schedule.
300 square feet that does not count against you
The city caps impervious coverage at 40%, but that cap bites on new builds, teardowns and big additions rather than on a standalone patio. On a lot of 10,000 sq ft or less an unenclosed, uncovered deck or patio can take up to 300 sq ft without counting toward it. Worth knowing before anyone talks you into a smaller patio than you want.

Yes. Base stone and pavers come in by wheelbarrow and the spoil goes out the same way. It is slower and it costs more, and the quote says so plainly rather than hiding it. What it does not change is the base — six inches compacted in layers, hand-hauled or not.
